The test kit includes a general questionnaire, a test kit, and a personalized vitamin D status report. Participants have the option to complete the questionnaire, or only have their vitamin D tested when they register.

Also included are detailed instructions, a lancet for finger-prick, sample collection paper, bandages, and a self-addressed shipping package for returning the sample.
